Assessment Process to Prepare for Independent Living

Living with a disability or while aging should not prevent a person’s dream or goal to live independently, safely, and with peace of mind. We put in place an Assessment process, made up of 3-4 Assessments, to help those we serve achieve their goals. When a Person's Support Team has their DDS Case Manager send us a referral, our triage team begins the process to identify and address barriers that would interfere with independence.

Independent Living Assessment

If the triage team agrees you are a candidate for Independent Living, the DDS Case Manager will be notified and an Independent Living Assessment will be sent to the person's Support Team (family, day provider, residential provider) to be completed.

Safety and Risk Assessment

The Safety and Risk Assessment is critical in the process. Our Director of Community Integration works with the person's Support Team to determine if they meet the safety requirements for Independent Living.

Transportation Assessment

The Assessment for Transportation will be reviewed by the team and options will be identified as needed.

Assistive Technology Assessment

The Assistive Technology (AT) Assessor will receive the completed Assessments and begin the funding request process. Once funding has been approved, the AT Assessor will begin the AT Assessment. Once completed and the AT equipment/software needed has been identified, we can move forward to the apartment living or begin a trial process with the assistive technology in their current living environment. This includes all areas including assessments, identification and purchase of equipment and software, installation, training, and the funding approval for ongoing monitoring fees.
